The work presented herein has been completed by myself with the following notable exceptions:

• The protocol, organisation and running of The FlexiScope Trial is attributable to hard work of Wendy Atkin and her team from Cancer Research UK (formerly Imperial Cancer Research Fund).
• DNA extraction of the Leeds samples was performed predominantly by the laboratory staff of the Cancer Genetics Department of St. James's University Hospital except for 280 samples that were commercially extracted by ScotLabs Plc.
• NAT1 genotype analysis was carried out by myself and the staff of GenoVar Diagnostics Ltd, Sittingbourne.  Heteroduplex analysis for the NAT1 genotype was performed entirely by GenoVar Diagnostics Ltd.
• Database management, data cleaning and processing was the responsibility of Rob Edwards and Rekha Patel of Cancer Research UK.
• Data analysis was collaborated upon by myself, Faye Turner, Jenny Barrett  and Tim Bishop using Stata software.
